Appointments

For basic health care needs, schedule an appointment with one of our primary care physicians or nurse practitioners. Primary care appointments must be made during regular appointment hours on Monday through Friday, 8 a.m. to 11: 30 a.m. and 1 p.m. to 4:30 p.m. On Thursday mornings, the clinic and phones open at 9 a.m. Appointments are usually available within a week of request.

If you are ill, injured or have an urgent medical need, you may walk into the Klotz Student Health Center without an appointment. A triage nurse can evaluate you and may be able to arrange for a same-day appointment or provide a referral to meet your needs.

First Appointment Information

New Klotz Student Health Center patients need to check in at the first-floor reception desk. Please remember to bring your CSUN student ID. To avoid delays and allow time to complete forms, plan to arrive at least 20 minutes before your scheduled appointment.

It saves time to complete your required new patient forms online through your myHealth patient portal.

1. Login to myHealth Patient Portal. Enter Username and Password.
2. Select the drop down arrows. Enter month, day, and year of birth.
3. Select Requirements and Forms.
4. Complete the following four forms before your first appointment:

  • SHC Treatment Agreement
  • SHC Acknowledgement of Fees
  • SHC Notice of Privacy Practices
  • SHC Additional Health

5. Select the Health History category to complete your health history form to the best of you knowledge.

6. When finished, select Log-out.

Scheduling an Appointment

Students can set up appointments over the phone by calling (818) 677-3666. When prompted on the automated system, select option one to schedule your appointments.

Students can currently schedule appointments online through their myHealth Patient Portal for the following services:

  • Acupuncture
  • Chiropractic Care
  • Eye Exam

When scheduling an appointment, please make sure you have your CSUN student ID number, username and password if using the online system, a description of your symptoms, the days and times you’re available for an appointment and, if applicable, the name of your preferred clinician. 

Telehealth Appointment Check In

Please have your photo/CSUN ID card readily available before beginning your telehealth visit. 

1. Login to myHealth Patient Portal. Enter Username and Password.

2. Select the drop down arrow. Enter month, day, and year of birth.

3. Select Appointments.

4. Select Appointment Check In (This function will appear 30 minutes prior to appointment time).

5. Complete the Telehealth questions that appear after you select check-in.

6. Join meeting and the clinician will reach out to you soon, please have your phone available if there are technical issues. 

If your clinician has not joined your meeting after 10 minutes, or you have any issues or questions about the check-in process please call our appointment line at (818) 677-3666.

Check In at the Student Health Center

Unless instructed otherwise, students should check-in for appointments with their CSUN Student ID at the first-floor reception desk of the Klotz Student Health Center. 

For in-person services including doctor appointments, laboratory appointments, radiology services, and pharmacy needs, please wear a mask if you are symptomatic or had COVID-19 exposure in the last 10 days prior to entering the Student Health Center. 

Cancelling an Appointment

To avoid a $5 no-show fee, please cancel appointments the business day prior to any scheduled appointment by 3:30 p.m. or as soon as possible. You can call (818) 677-3666 or cancel directly through your patient portal. When you cancel an appointment, that appointment immediately becomes available to other students. Cancelling appointments leads to more patient visits and shorter wait times for appointments.

Cancelling an Appointment Online

  1. Log in to the myHealth Patient Portal.
  2. Select Appointments.
  3. Select the appointment you would like to cancel.
  4. Select Cancel Appointment.
  5. Select a reason.
  6. Select OK.

Cancelling an Appointment by Phone

Call the Klotz Student Health Center at (818) 677-3666. If you leave a message, include your name, CSUN Student ID number, appointment date, and appointment time.

Accessing Secure Messages from the Klotz Student Health Center

To access your secure messages, please complete the SHC Notice of Privacy Practices form.
