Authorization to Release Information
Submit this form to authorize the Financial Aid & Scholarship Department to release student or parent information to your parent. This form must be completed every year and submitted in-person.

2025-2026 Status Change Notification Form
Submit this form to report enrollment status changes for the fall and/or spring term. You can access this form by going to the CSUN home page and logging in to the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. It may take up to 4 weeks for this request to be processed.

Summer Federal Direct Request Form
If you have not borrowed the maximum loan amounts for 2025–2026, you may submit a Summer 2026 Federal Direct Loan Request. To access the form, log in to the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. Note: Requests will not be processed until you are enrolled at least half-time. Processing may take up to 3 weeks.

Summer Federal Direct Graduate PLUS Request
To request a Summer 2026 Graduate PLUS loan, log in to the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. Note: Requests will not be processed until you are enrolled at least half-time. Processing may take up to 3 weeks.

Summer Federal Direct Parent PLUS Loan Request Form
Use this form to apply for the Federal Direct Parent PLUS Loan for undergraduate students for Summer 2026. For more information, visit the CSUN Federal PLUS Loan website. Note: Submit the Parent PLUS request after financial aid has been awarded. Log in to the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. Upload the form using the Financial Aid Document Upload under Financial Aid Forms.

The deadline to submit a Summer 2026 loan request is:

  • Session 1 & Session 3: August 13, 2026
  • Session 2: June 30, 2026

Note: The deadline to submit loan requests for Fall 2025 and Spring 2026 has passed.
