Booking Request Form

 INSTRUCTIONS:

Holding Tickets: To "temporarily hold" your tickets, please complete and submit the form below. Please note however, your tickets are not guaranteed until the deposit/payment has been received. To complete the reservation process and release the temporary hold, you can either pay in full or provide a non-refundable booking deposit of 10% of your total ticket purchase. 

Payments: Payments can be mailed to CSUN, Teenage Drama Workshop, Attention: Alicia Lawrence, 18111 Nordhoff St., Northridge, CA 91330-8320. Please make checks payable to CSUN, Teenage Drama Workshop and include TADW in the memo. Payments can also be made via credit card by contacting Alicia Lawrence at (818) 677-3088.

Note: The number of tickets can be estimated at the time of booking, but final numbers and payment of balance are required no later than July 1st. We cannot guarantee seating availability for balances not paid in full by July 1st. Please note, we may not be able to accommodate last minute additions on the day of your scheduled reservation.

*There are NO REFUNDS on purchased tickets.
