Inventory Survey

Why?
The survey below is designed to be a convenient method by which faculty and staff can report their community service, engagement and outreach efforts. The university can inventory the responses, and communicate the results of this inventory to local, regional and statewide stakeholders.
Faculty and staff can view the inventory and the associated maps to find others on campus who may be participating in similar activities, working in specific neighborhoods or about grant activity where synergies through cooperative action might be possible. Faculty and staff, applying for grants may also find this inventory and associated maps useful tools as they apply for grants and find themselves called to account for "needs and capabilities".
Check with your department chair or personnell committee for information about how the data you report below may be incorporated into your promotion, retention and tenure materials.
Instructions
The survey questions are largely self-explanatory, but a few helpful suggestions are listed below.
Before you start:
- Note that a few questions are mandatory (name, college, department, etc.).
- Please try to be specific as possible when entering the address or location of your service activity - this helps the software place a marker on the map to indicate where your service activity took place.
- If you have an extended description of your efforts on a webpage, please have the URL ready to copy / paste into this form.
- After clicking the submit button at the end of the survey, you will be provided a link that will allow YOU to modify your submission after it has been submitted. You should bookmark that page and save the link in a separate document so you can amend your information at your leisure.
- There's room at the end of the survey to add comments to help us better gather this information.
