How to Submit COVID-19 Vaccine Self-Certification Via SOLAR

CSU COVID-19 Vaccination Policy

Pursuant to the CSU COVID-19 Policy, all individuals who access campus/programs must be fully vaccinated against the COVID-19 virus.

To comply with the CSU COVID-19 Vaccine Self-Certification requirement, please follow the directions. Office of the Registrar Records and Registration will review and process your submission within 1 business day.

Access your COVID-19 Vaccine Self-Certification Form

  1. Open the CSUN webpage and select the MyCSUN link.

  2. Select Sign In to login to CSUN Portal.

  3. Select COVID-19 Resources in the Student Services tab.

  4. Select Vaccine Self-Certification to access the COVID-19 self-certification form.

  5. Review Section I: Self-Certification of COVID-19 Vaccination Status and select one option that applies to you.

Based on your self-certification response, please review the relevant tabs below.

Submit a COVID-19 Vaccine Self-Certification: Fully Vaccinated and Booster Dose Information

  1. Review Section I: Self-Certification of COVID-19 Vaccination Status and select one of the following options that applies to you.

    1. If you certify your status is FULLY VACCINATED: Complete documentation of COVID-19 Vaccination information: Provide Manufacturer of your COVID-19 vaccine, Date Received 1st Dose, and Date Received 2nd dose.
    2. Provide location information of vaccination received: Facility (if known), City, State/Province/Region, and Country.
    3. Provide BOOSTER information: Date Received and Manufacturer.

  2. Review Section II: Self-Attestation of Accuracy of Information Provided, and select the checkbox to provide your confirmation.

  3. Review Section III – Attachments, to upload a copy of your COVID-19 Vaccination Record Card (this is an optional upload).

    1. To submit a copy of your COVID-19 Vaccination Record Card and Booster Record Card, select Attach to open the file upload window.

    2. Select Choose File to browse for your COVID-19 Vaccination document. Find your document and select Upload to submit your COVID-19 Vaccination Record Card.

  4. Select Submit button to complete the Self-Certification process.

  5. Once submitted, a confirmation message and date of submission appears: "Thank you for submitting your COVID-19 Vaccine self-certification. Your form will be processed within 1 business days of submission."

  6. Select Finish to return to the Student Center.

Submit a COVID-19 Vaccine Exemption Request

  1. Review Section I: Self-Certification of COVID-19 Vaccination Status and select one of the following options that applies to you.
    1. If you certify you qualify for a medical and or religious exemption, please follow steps 2 through 6.

  2. Review Section II: Self-Attestation of Accuracy of Information Provided, and select the checkbox to provide your confirmation.

  3. Review Section III – Attachments, to upload a copy of your religious or medical exemption request form.

    1. To submit a copy of your religious or medical exemption request form, select Attach to open the file upload window.

    2. Choose File to browse for your religious or medical exemption request form. Find your document and select Upload to submit your religious or medical exemption request form.

  4. Select Submit button to complete the Self-Certification process.

  5. Once submitted, a confirmation message and date of submission appears: "Thank you for submitting your COVID-19 Vaccine self-certification. Your form will be processed within 1 business days of submission."

  6. Select Finish to return to the Student Center.

View Your COVID-19 Vaccination Requirement Hold

Holds are "indicators" on your student record that require your attention. Some holds prevent you from enrolling in classes until you clear them. These are called registration holds. To review details for common advisement, financial, immunization and other holds, visit registration holds.

Check for registration holds well before your enrollment appointment each semester. View holds online in two locations:

  • Registration holds only - Checklist section, Incomplete Tasks
  • All holds - Student Center
  1. Select CSUN Portal under the MyCSUN icon on the CSUN homepage to login to your CSUN Portal.

  2. All registration holds are listed in the Incomplete Tasks section of your Checklist.

  3. In the Top Tools section, select Student Center to view all holds.

  4. The Student Center displays all holds in the Holds section. Select Details for more information on any registration holds in your student account.

  5. The Item List displays the student registration holds for the semester. Select COVID-19 Attestation Required for more information about the registration hold.

  6. The Hold Item displays information regarding the term of the hold, reason for the hold, contact information for the appropriate department, and brief instructions to clear the hold. Select Return to return to the Student Center.
