Principal Investigators and Research Administrators may now download most forms required for the administration of sponsored programs. The forms can be found at the Sponsored Programs website, http://www.csun.edu/sponsoredprograms/forms.htm .

In addition to previously posted forms, the following forms are now also available for download at the website:

- Check Request
- Purchase Order
- Hourly Time Sheet
- Salary Payment Authorization
- Attendance Report for Salaried Employees
- Faculty Payment Request
- Request for Independent Contractor

For these newly available forms, it is very important that you submit the correct number of signed copies required for each type. The number of copies is noted at the bottom of each form in the section ‘Instructions for Requestor,’ and is as follows:

- Check Request:	2 copies
- Purchase Order:	5 copies  
- Hourly Time Sheet:	2 copies
- Salary Payment Authorization:	2 copies 
- Attendance Report for Salaried Employees:  1 copy
- Faculty Payment Request:	2 copies 
- Request for Independent Contractor:	2 copies

You may also continue to use the printed version of these forms, which are available at The University Corporation front desk.
