How to Submit Immunization Documents via Student Portal (myhealth.csun.edu)

  1. Be prepared! Gather all the information you’ll need to complete your requirements online.
  2. Login to the MyHealth Portal.
  3. Once you are logged in, select Medical Clearances on the left menu selection.

  4. Select the Update button for Immunization Record.

  5. The Immunization Record box appears on the screen:
    • If you have physical copies of your immunization records: Please upload the images or PDF files of your immunization records, laboratory tests (titers), and/or recent chest X-ray here.

      • Once your immunizations records are uploaded, select Save to submit your immunization records.

    • If you do not have physical copies of your immunization records: Select the Download button to access the "California State University, Northridge Immunization Requirements" form.

      • Please include your name and CSUN Student ID number on the form.
      • Have your healthcare provider complete the immunization records and sign the document.
        • Upload the completed form.

      • Select Save to submit your completed "California State University, Northridge Immunization Requirements" form.

  6. Your records will processed within 5 business days upon submission.
  7. When finished, select Log Out.
