Family PACT
Our free and low-cost services and programs are available to currently enrolled CSUN students. We welcome students to visit the Klotz Student Health Center for a free consultation with a clinician, nurse, or clinical health educator to explore their health and wellness needs.
Health insurance is not required to access Klotz Student Health Center services.
The Klotz Student Health Center participates in the Family PACT program that can help eligible students receive free or low-cost sexual and reproductive health services.

Family PACT is a program administered by the California Department of Health Care Services (DHCS), Office of Family Planning (OFP). If you qualify, Family PACT covers family planning and related services (sexual and reproductive health) at no cost.
What does FPACT cover?
- Family PACT covers most sexual and reproductive health services only. Covered services include, but are not limited to, various birth control methods, sexually transmitted infection (STI) testing and treatment, and HIV testing.

To apply for Family PACT, please call us at (818) 677-3666 to schedule a Family PACT appointment with our lab reception. Our lab reception will assist with the creation and completion of your application and provide education about Family PACT-covered services. Please bring a photo ID.
The Family PACT enrollment hours at the Klotz SHC are Monday through Friday from 8:30 am to 11:30 am (the last appointment is at 11:15 am) and 1:30 pm to 4:30 pm (the last appointment is 4:15 pm).
If you have not previously applied for Family PACT online, please call us at (818) 677-3666 to schedule a Family PACT appointment with the lab reception.
If you have previously applied for Family PACT online, please follow the instructions below:
- Visit the Family PACT Portal and select Client Account Login.
- Login to your account and select Recertify.
- If you are eligible or qualify, take a screen shot of your complete Family PACT card, including your full name, HAP I.D. number, and eligibility dates.
To upload your Family PACT card to the MyHealth Patient Portal:
- Login to the MyHealth Patient Portal with your CSUN credentials.
- Select Insurance Card.
a. Please note: We only accept Family PACT card for laboratory services. - Select Add New Card.
- Select Upload Front Image.
a. We accept the following file types: PDF, PNG, GIF, JPEG, and JPG. - Select Save to complete your Family PACT card upload.
The Family PACT card must be uploaded at least 24 hours prior to your scheduled sexual or reproductive health appointment.

IMPORTANT: The Family PACT card must be uploaded 24 hours prior to your sexual or reproductive health appointment. If the card is not uploaded at least 24 hours prior to your appointment, you are responsible for any fees related to the visit.
