Report an Accident

Accidents may involve employees, University vehicles, students or campus visitors, in any combination.  Reports may have to go to various individuals, campus departments or California State agencies.  See the following for guidance.

Vehicle Accident:

Where and how to report a vehicular accident depends on what you are driving.  In any case, prompt reporting is essential to preserve your rights. Only authorized drivers are allowed to drive State (University) vehicles.  The following is intended to present guidelines for accident reporting.

IF:

  • You are an authorized employee or volunteer and you are driving a State (University owned) vehicle, you should report to: your supervisor, the State Office of Risk Management and Insurance (ORIM) by telephone at (916) 376-5302 or by FAX (916) 376-5277 and University Risk Management (818) 677-2079.
  • You are an authorized employee or volunteer and you are driving a State (University) rented vehicle, you should report to: your supervisor, the State Office of Risk Management and Insurance (ORIM) by telephone at (916) 376-5302 or by FAX (916) 376-5277, University Risk Management (818) 677-2079 and the Rental Agency.
  • You are an authorized employee or volunteer and you are driving a State (University) leased vehicle, you should report to: your supervisor, the State Office of Risk Management and Insurance (ORIM) by telephone at (916) 376-5302 or by FAX (916) 376-5277, University Risk Management (818) 677-2079 and the Leasing Agency.
  • You are an authorized employee or volunteer and you are driving your personal vehicle, you should report to: your supervisor, the State Office of Risk Management and Insurance (ORIM) by telephone at (916) 376-5302 or by FAX (916) 376-527, University Risk Management (818) 677-2079, the California State DMV and your auto insurance company.

If you drive your personal vehicle on University business and are involved in an accident, you are required to follow normal DMV procedures for reporting the accident if anyone is hurt or there is property damage in excess of $750.

ORIM will advise State drivers which required forms to file.  As a University driver, in case of an auto accident you are urged to read the procedures titled “OK, so you had a motor vehicle accident, so now what happens?

Employee Accident:

Any employee injured on the job must report the injury immediately to his/her supervisor.

The injured employee’s supervisor completes the Supervisor’s Accident Investigation Report for Employee Injury form for every reported employee incident or injury/illness occurring on the job, including minor injuries.  It should be completed through a one-on-one discussion of the circumstances that caused the accident so that workplace conditions or work practices that contributed to the accident can be corrected.

In addition to completing this form, the supervisor must contact EH&S (Ext. 2401, Fax 5853) to report the accident as soon as possible, but no later than 8 hours.  Copies of the completed form should be sent to both the Environmental Safety & Health Department (MD 8284) and Human Resources Services (MD 8229).  Retain original in department files.

Also, a State of California “Employee’s Claim for Workers’ Compensation Benefits form, DWC Form 1, must be given to the injured employee within 24 hours of the employer’s knowledge of the work  injury/illness, according to state law.  The form spells out the benefits an injured employee may be entitled to receive.  To meet the time requirement, it may be necessary to mail the form to the injured employee.  It is up to the injured employee to complete the form and return it to management if medical treatment is necessary.  Otherwise, the signed form does not have to be returned to the supervisor.

Student, Vendor, Visitor Accident:

Accident Report (Other than Motor Vehicle)

The above report should be submitted to EH&S by the instructor, supervisor, or other University employee having knowledge of an accident whenever a student, vendor, or campus visitor is injured on University property or during a University-sponsored activity and/or if personal property damage is incurred.  All injuries, other than first aid, should be reported.
