Students must complete the following:
Additional Forms if Driving (even to the airport) and/or Flying
- Student Authorization to Operate Privately Owned Vehicle (.pdf)
- Copy of valid car Insurance
- Copy of valid driver's license
- Student Air Travel Voluntary Participation (.pdf)
Please note the Department should keep the original Additional forms for students and provide the Dean's office with a copy in case of emergency.
Students Traveling Alone
Student must provide the following:
- Approval of Travel Request (.pdf)

- Documentation why the student is traveling alone (Ex. Is it required for class, internship, etc.)
