 IMPORTANT INSURANCE REQUEST FORM REQUIREMENTS

  • “TODAY’S DATE,” means the date the form is completed and submitted. 
  • If the form is NOT COMPLETE, it will NOT BE SUBMITTED.
  • Forms must be submitted at LEAST TWO WEEKS PRIOR TO DATE OF FIRST SHOOT DATE!
  • “The top portion of Attachment “A” must be completed by the production group” ADD for all insurance requests.
  • Continue with remainder for equipment rental, an ITEMIZED LIST OF EQUIPMENT must be attached and must include the REPLACEMENT VALUE for each item (itemized) and TOTAL REPLACEMENT VALUE, THE PICK-UP DATE(S) AND THE RETURN DATE(S) AND MUST BE ON VENDOR LETTERHEAD.
