The LifeMatters@CSUN – Employee Assistance Program is a benefit offered to California State University, Northridge employees and their household members. It provides free, confidential support 24/7, including short-term counseling, guidance on personal or work issues, and referrals to local resources for things like stress, relationships, family needs, work/life balance, and more. Services are available by phone and online as part of the university’s work/life resources for staff well-being.

Staff Performance Evaluations –

Performance Evaluation - Staff (Units C99, 1, 2, 5, 6, 7, 8, & 9)    (Word Version)

This form is to be used for the evaluation of support staff employees except Unit 4, Academic Support, which uses the “Performance Evaluation for Staff in Unit 4, Academic Support only” form. Evaluations are conducted during the Employee's probationary period and annually thereafter. Please refer to the applicable Collective Bargaining Agreement for more information on the timing of performance evaluations. If space is inadequate, the Evaluator may use the additional fields provided on Page 4 of this form. Additional information or supporting documents as well as the Employee’s self-evaluation may be attached on Page 3 of this form.

Performance Evaluations should be completed by the Employee’s Appropriate Administrator (Evaluator). After filling out the Performance Evaluation form, select the “Click to Sign” button at the bottom of the page. The button will not appear until you have completed all required fields. Next, you will be asked to enter the email addresses (CSUN email addresses only) for the remaining recipients of this form, listed below.

  1. Reviewing Officer (the Evaluator’s Appropriate Administrator)
  2. Employee being evaluated

You will also be asked to enter your email address (please use your CSUN email address only) in order to authenticate your identity. You will receive an email from Adobe Sign, asking you to select a link in order to confirm your email address. Please complete this step or your evaluation will not successfully submit.

Recommendations for actions that should be completed prior to submitting this form:

  1. The Appropriate Administrator may request a draft from a designated Evaluator regarding the Employee’s job performance. Only the Appropriate Administrator shall submit a draft evaluation for the Employee’s review, input, and discussion. Prior to meeting with the Employee to discuss the draft evaluation, the Appropriate Administrator may request the Employee complete a voluntary self-evaluation, which will also be discussed during the performance evaluation meeting. Employees in Units 2, 5, 7, and 9 shall be given up to 10 work days to review a draft evaluation and provide input. Employees in Unit 6 shall be given five (5) workdays. The Appropriate Administrator shall consider the input in preparing the final evaluation. The Employee’s self-evaluation can optionally be attached by the Evaluator on Page 3 of this form.
  2. Provide the Employee with a draft copy of the Performance Evaluation prior to a performance evaluation meeting. During the meeting, the Appropriate Administrator should encourage the Employee to discuss his/her opinions and observations regarding the content of the evaluation.
  3. Upon completion of the evaluation meeting, the Appropriate Administrator (Evaluator) will submit the Performance Evaluation using this online form.

Performance Evaluation - Unit 4    (Word Version)

This form is to be used for the evaluation of employees in Unit 4, Academic Support. For all other employees, please use the “Performance Evaluation for Staff in Units 1, 2, 5, 6, 7, 8, and 9” form.

  1. Prepare an initial draft of the Performance Evaluation in consultation with the Reviewing Officer. Prior to meeting with the Employee to discuss the draft evaluation, the Evaluator may request the Employee complete a voluntary self-evaluation, which will also be discussed during the performance evaluation meeting. The Employee’s self-evaluation can optionally be attached by the Evaluator on Page 3 of this form.
  2. Provide the Employee with a draft copy of the Performance Evaluation prior to a performance evaluation meeting and inform the Employee of the date, time, place and purpose of the evaluation meeting. The Employee shall have fourteen (14) days to review the draft evaluation, provide verbal feedback, and submit a rebuttal (if any) to the evaluator before the evaluation is finalized. During the meeting, the Evaluator should encourage the Employee to discuss his/her opinions and observations regarding the content of the evaluation.
  3. Upon completion of the evaluation meeting, the Evaluator will submit the Performance Evaluation using this online form.

This form is to be used for the performance evaluation of Management Personnel Plan (MPP) employees.

The form requires the Overall Evaluation rating for the MPP Employee and the attachment of a detailed evaluation of employee performance and learning. An attachment field is located on the form.
