Special Course Request Form

The "Special Course Request Form" allows students to request additional course(s) or modification(s) to an existing schedule for an upcoming semester.  

This request form is available to students October 01, 2022 through November 30, 2022 for the Spring 2023 semester.

This request form is used to facilitate the organization of student requests for the Office Staff and Department Chair.  

Submission of this form is not a guarantee you will receive the course(s) you have requested. 

If you are currently enrolled in a CD course, you are encouraged to continue to attending course meetings and complete assignments until you receive a response from the Department.
