Parent PLUS Loan Request Form 
Use this form to apply for the Federal Direct Parent PLUS Loan for Undergraduate students. For more information, visit the CSUN Federal PLUS Loan website. 

Note: Submit the Parent PLUS request after financial aid has been awarded. Log in to the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. Upload the form using the Financial Aid Document Upload under Financial Aid Forms.

Graduate PLUS Loan Request Form 
Graduate PLUS loans are available to borrowers who meet “legacy” provisions as established by the new federal law, the One Big Beautiful Bill Act. Graduate and Professional students who meet the qualifications can submit this form to apply for the federal direct Graduate PLUS loan. You can access this form by logging in to the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. It may take up to 4 weeks for this request to be processed.

Federal Direct Loan Request Form
Submit this form to apply for the federal direct loan. You can access this form by logging in to the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. It may take up to 4 weeks for this request to be processed.

Authorization to Release Information
Submit this form to authorize the Financial Aid & Scholarship Department to release student or parent information to your parent. This form must be completed every year and submitted in-person.

2026-2027 Status Change Notification Form
Submit this form to report enrollment status changes for the fall and/or spring term. You can access this form by going to the CSUN home page and logging in to the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. It may take up to 4 weeks for this request to be processed.
