Loan Documents
Summer Federal Direct Request Form
If you have not borrowed the maximum loan amounts for 2022-2023, you may submit a Summer 2023 Federal Direct Loan Request. To access the loan request, log into the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. Note - Your request will not be processed until you are enrolled at least half time. It may take up to 3 weeks for this request to be processed.
Summer Federal Direct Graduate PLUS Request
To request a Summer 2023 Graduate PLUS Loan, log into the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. Note - Your request will not be processed until you are enrolled at least half time. It may take up to 3 weeks for this request to be processed.
Summer Federal Direct Parent PLUS Loan Request Form **
Select this form to apply for the federal direct parent loan for undergraduate students (PLUS) for summer 2023. For information about the parent loan program, go to http://www.csun.edu/financialaid/federal-plus-loan (The request for Parent PLUS should be submitted after being awarded financial aid. Log into the CSUN portal and upload this form using the dropdown menu under My Finances.
The Deadline to submit a summer loan request is as follows:
Summer 2023 Session 1 & Session 3 Deadline: August 8, 2023
Summer 2022 Session 2 Deadline: June 27, 2023
Note: The deadline to submit a loan request for fall and spring 2022-2023 has passed.

Income Appeal Form
This form is used to apply for an Income Appeal for the 2022-2023 academic year. Do not submit this form until you read the important information and FAQs located at https://www.csun.edu/financialaid/appealsspecial-circumstances.
Note: Do not submit an Income Appeal if the following applies to you. Submitting an appeal will NOT change your aid eligibility:
- Your 2022-2023 Expected Family Contribution (EFC) is $0.
- You are a 2nd BA degree student who has been offered $5,500 in a Federal Direct Subsidized Loan.
- You are a 2nd Credential student, or are enrolled in an Intern Credential program and were offered$5,500 in a Federal Direct Subsidized Loan.
- You are in a Master’s degree program.
Cost of Attendance Appeal (COA) Form
This form is used to apply for a COA Appeal for the 2022-2023 academic year. Do not submit this form until you read the important information and FAQs located at https://www.csun.edu/financialaid/appealsspecial-circumstances.
Dependency Override Request
Students who are considered dependent per federal/state definition must provide parental information on their financial aid application. In extraordinary and documented cases, a financial aid administrator may override a student’s dependency status for the purpose of applying for financial aid. Go to https://www.csun.edu/financialaid/appealsspecial-circumstances and read the information FAQs if you believe you have extraordinary circumstances which warrant a review of your dependency status for 2022-2023.

2022-2023 Verification of Non-filing
Submit this form if we have requested you/spouse/parent to submit a 2020 verification of non-filing from the IRS and you have not been able to obtain that verification from the IRS or any tax authority.
2022-2023 Status Change Notification Form
Submit this form to report enrollment status changes for fall and/or spring term. You can access this form by going to the CSUN home page and log into the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. It may take up to 4 weeks for this request to be processed.
Authorization to Release Information
Submit this form to authorize the Financial Aid & Scholarship Department to release student or parent information to your parent. This form must be completed every year.
2022-2023 Confirmation of Update to Citizenship
Submit this form to confirm that you have updated your U.S. citizenship status with the Social Security Administration. You must also present, in person, to the Financial Aid & Scholarship Department your original citizenship document as proof of your U.S. citizenship status. Do not send original citizenship documents by mail. For additional information, refer to How to Submit Proof of U.S. Citizenship guide.
2022-2023 Confirmation of Eligible Noncitizen Status
Submit this form to confirm you are in an eligible noncitizen immigration status for financial aid purposes. Do not submit original immigration status documents by mail. For additional information, refer to How to Submit Proof of Eligible Noncitizen Status guide.
2022-2023 Statement of Educational Purpose
Submit this form in person to the Financial Aid and Scholarship Department for verification of identity. As required by the U.S. Department of Education, you must present a valid government-issued photo ID and sign the Statement of Educational Purpose attesting that the federal financial assistance you receive will be used for educational purposes only. If you are unable to appear in person, you must sign the statement, have a notary public sign it to confirm that you appeared and presented the ID confirming your identity. Once completed, send the notarized form with a copy of your ID via regular mail using the address on the form.
2022-2023 Loan Default Documentation
Submit this acknowledgement form if you are currently in a student loan default status and qualify for the Fresh Start initiative announced by the U.S. Department of Education. Only submit this form if you defaulted on your student loans before March 13, 2020.
