Loan Documents

Federal Direct Parent PLUS Loan Request Form
Submit this form to apply for the federal direct parent loan for undergraduate students (PLUS). For information about this parent loan program, go to http://www.csun.edu/financialaid/federal-plus-loan (Do not submit this form until you have been awarded your financial aid)

Federal Direct Graduate PLUS Loan Request Form

You can access this form by logging into the myNorthridge portal and going to the Financial Matters tab.  Log in and complete the electronic form.  Note - Your request will not be processed until you are enrolled at least half time. It may take up to 4 weeks for this request to be processed.

Federal Direct Loan Request Form 
DO NOT use this form if you can accept your Direct Loan online (see How to Accept a Loan tutorial)

Income Appeal Forms

Income Appeal Form
Submit this form to apply for an income appeal for the 2020-2021 academic year.

Cost of Attendance Increase Appeal Form
Submit this form to apply for an increase to your Cost of attendance for the 2020-2021 academic year.

Note: Please do not submit an appeal if the following applies to you.  Submitting an appeal will NOT change your aid eligibility:

  • You filed your FAFSA/CADAA after March 2, 2020
  • Your 2020-2021 FAFSA Expected Family Contribution (EFC) is $0.
  • You are a 2nd BA degree student who has been offered $5,500 in a Federal Direct Subsidized Loan.
  • You are a 2nd Credential student, or are enrolled in an Intern Credential program and were offered$5,500 in a Federal Direct Subsidized Loan.
  • You are a 2nd Master’s degree student.
  • You are a Master’s degree student and you have been awarded a State University Grant.

Other Forms and Documents

2020-2021 Verification of Non-filing
Submit this form if we have requested you/spouse/parent to submit a 2018 verification of non-filing from the IRS and you have not been able to obtain that verification from the IRS or any tax authority.

2020-2021 Status Change Notification Form
Submit this form to report enrollment status changes for fall and/or spring term. You can access this form by logging into the myNorthridge portal and going to the Financial Matters tab.  Log in and complete the electronic Status Change Notification.  It may take up to 4 weeks for this request to be processed.

Authorization to Release Information
Submit this form to authorize the Financial Aid & Scholarship Department to release student or parent information to a third party. This form must be completed every year.

2020-2021 Confirmation of Update to Citizenship 
Submit this form to confirm that you have updated your U.S. citizenship status with the Social Security Administration. You must also present, in person, to the Financial Aid & Scholarship Department your original citizenship document as proof of your U.S. citizenship status. Do not send original citizenship documents by mail.  For additional information, refer to How to Submit Proof of U.S. Citizenship guide.

2020-2021 Selective Service Registration  
Submit this form to notify the Financial Aid & Scholarship Department of your Selective Service registration status. Male students between the ages of 18 and 25 are required to register with Selective Service before financial aid eligibility can be determined. You must attach proof of your Selective Service registration or proof of your exemption status. Log in to the myNorthridge Portal, go to the SOLAR Student Center and review your To-Do List to see if the form is requested.  For additional information, refer to How to Submit Proof of Selective Service Registration guide.

2020-2021 Statement of Educational Purpose 
Submit this form so that the Financial Aid & Scholarship Department can verify your identity and collect a Statement of Educational Purpose as required by the U.S. Department of Education. You must appear in person at the Financial Aid & Scholarship Department, present a valid government-issued photo ID and sign a statement of Educational Purpose attesting that the federal financial assistance you receive will be used for educational purposes only. If you are unable to appear in person, you must sign the statement, have a notary public sign it to confirm that you appeared and presented the ID confirming your identity, and submit it with a copy of your ID.

Government ID Affidavit
Submit this form if unable to present proof of your eligible noncitizen document(s) in person.   You must sign the statement, have a notary public sign it to confirm that you appeared and presented the ID confirming your identity, and submit it with a copy of your ID.
