2019 CSUN Jolene Koester Team Award Nomination Form

The Jolene Koester Team Award is bestowed on teams of staff members who collaborate on a project, process, or other significant initiative that brings about radical, positive change that improves performance and/or image, or increases productivity, and/or reduces costs.

Eligibility:

Teams should consist of a minimum of three individuals. Work groups, departmental units, or cross-functional teams, and cross-departmental
groupings are eligible.

Please provide information pertaining to the Team nominated, You (the Nominator), and the Nominee's Supervisor.

NOMINATION:

Please list the first and last name, and department of each team member nominated. e.g.: Anabel Smith, Physical Plant Management; Bernard Jones, Biology Department; Cameron Lee, Student Affairs. If additional members, please include information in the "optional" comments at the end of the form.

Provide information as requested as to how this team/project meets the criteria for the award.

It is recommended that you use the preview of the nomination form provided on the Staff Service Awards page to prepare your responses so that you may paste them into the form in one sitting.

After you successfully submit your nomination, you will receive an email with which you can view and print a copy of your nomination. If you do not receive an email, you have not successfully submitted your nomination.
