How to Enroll

Enrollment for 2015 will start the first week in March.

Your child is not enrolled until you have completed ALL three of the following steps:

  1. Complete Online Registration Form

    See our Register page for the form

    Get scholarship application (.pdf)

  2. Make Payment

    You may make an online payment during the registration process with a credit card (Master Card, Visa, Discover Card), debit card or e-check.

  3. Return Four Essential Forms

    Download, print, sign and return the four essential forms to the address below within two weeks of registering.

    • Form #1 – "Informed Consent, Waiver & Release"
    • Form #2 – "Authorization to Treat a Minor"
    • Form #3 – "Authorization to Pick-Up Form"
    • Form #4 – "Late Pick-Up and Fees"
