How to Enroll
Enrollment for 2015 will start the first week in March.
Your child is not enrolled until you have completed ALL three of the following steps:
Complete Online Registration Form
Get scholarship application (.pdf)
You may make an online payment during the registration process with a credit card (Master Card, Visa, Discover Card), debit card or e-check.
Return Four Essential Forms
Download, print, sign and return the four essential forms to the address below within two weeks of registering.
- Form #1 – "Informed Consent, Waiver & Release"
- Form #2 – "Authorization to Treat a Minor"
- Form #3 – "Authorization to Pick-Up Form"
- Form #4 – "Late Pick-Up and Fees"
