Late Change Forms (required)

Students whose situation is serious and compelling for which there is no viable alternative may request a late schedule change by filing the appropriate form(s) for their student group below. These forms are also available at Student Forms.

NOTE: Your entries will not be saved if you complete a fillable PDF form in a web browser! Be sure to download and save PDF forms to your device to fill them out. Helpful: How to File a Form


Undergraduate and Second Bachelor's Students

Add or Drop, Undergraduate

During Weeks 4-12 of fall and spring semesters:

Medical Withdrawal, Undergraduate

As soon as classes begin, complete the Student Consent and Health Care Provider Guidelines (PDF) release form.

In Weeks 1-4, during online registration, complete the Medical Withdrawal Verification in Weeks 1-4 (PDF) to request a medical withdrawal notation on your student record (optional).

After Week 4, submit two forms together:

  1. Late Change in Academic Schedule for Undergraduate and Second Bachelor’s Students (PDF) form.
    and
  2. Student Consent and Health Care Provider Guidelines (PDF) release form.

Graduate: Master's, Doctoral and Credential Students

Medical Withdrawal, Graduate

Request any type of medical withdrawal any time after classes begin by directly contacting the Office of Graduate Studies and completing the form Late Change in Academic Schedule for Graduate Students Only (PDF)

Be sure to fill out the “Medical Withdrawal for Graduate Students: Consent and Health Care Provider Guidelines” attachment included in the form packet immediately above.
