Late Change Forms (required)

Students whose situation is serious and compelling for which there is no viable alternative may request a late schedule change by filing the appropriate form(s) for their class level — undergraduate or graduate — below. These forms are also available at Student Forms.

Undergraduate and Second Bachelor's Students

Add or Drop, Undergraduate

The weeks below refer to the fall and spring semesters. Find exact dates in the Registration Calendar as well as links to winter and summer term information.

Week 4:

  • ADD: No forms are required to add a class. Add online with a permission number from the instructor.
  • CHANGE GRADING BASIS: 
  • DROP/SWAP: Your class level determines whether approval is required to drop/swap in Week 4.
    • Undergraduate and second bachelor's students: You may drop/swap online using self-service registration through the end of Week 4, as defined in the Registration Calendar.

Medical Withdrawal, Undergraduate

If you not need CSUN to keep a record of your medical withdrawal in the first four weeks of classes, no forms are required. Simply drop the class in self-service registration.

However, if you'd like a full or partial medical withdrawal notated on your CSUN student record at any time after weekday classes begin, complete the following two forms:

  1. Late Change in Academic Schedule for Undergraduate and Second Bachelor’s Students (PDF) form.
    and
  2. Student Consent and Health Care Provider Guidelines (PDF) release form.

Graduate: Master's, Doctoral, Credential & University Certificate Students

Add or Drop, Graduate

During Weeks 4 through 12 of fall and spring semesters:

  • ADD: To add a class, request a permission number from the instructor and enroll in the class using self-service registration. No form is required. The deadline to add with permission is the end of Week 12.
  • DROP/SWAP: The last day to drop or swap a class using self-service registration is the end of Week 3 (usually the third Friday of classes). To request approval to drop or swap after Week 3, complete the Late Change in Academic Schedule (PDF) form available at Graduate Studies Evaluations: Forms.

After Week 12:

  • To request any kind of late schedule change during Week 13 and thereafter, including retroactively, complete the Late Change in Academic Schedule (PDF) available at Graduate Studies Evaluations: Forms.

Medical Withdrawal, Graduate

To request any type of medical withdrawal any time after weekday classes begin, contact the Office of Graduate Evaluations and complete the “Late Change in Academic Schedule” form, available at Graduate Evaluations: Forms.

Be sure to fill out the section “Medical Withdrawal for Graduate Students: Consent and Health Care Provider Guidelines” of the Late Change in Academic Schedule (PDF) immediately above. The two forms are combined into one file.
