Sport Clubs Participation Checklist
Students that have made their respective Sport Club for the Fall 2022 semester must complete additional paperwork in order to be eligible to play. The deadline to complete paperwork is October 28, 2022, and may be sooner if your club is cleared to begin competition or performances.
All players must complete:
- Waiver
- Emergency Card
- Code of Conduct and Academic Release
- Submit copies of Student ID and Driver’s License to the Sport Clubs Office
New Players:
- Complete New Participant Medical Forms and turn into the AT Office
- Schedule Baseline Concussion Test with Athletic Training (High Risk Sports Only) Physical (High Risk Sports Only)
Returning Players:
- Complete Returning Participant Medical Forms and turn into the AT Office
