How to use access keys
This information applies to pages in the CSUN template system.Windows-press ALT + an access key. Macintosh-press CTRL + an access key.

First Appointment Information
For your first visit to the Klotz Student Health Center, check in at the first-floor reception desk. Remember to bring your Cal State Northridge Student ID.
To avoid delays, plan to arrive at least 10 minutes before your scheduled appointment to allow time to complete several required forms. To save time, you can download, complete, and bring the following forms to your appointment.
- Health History Form*
- General Consent for Treatment Form
- Consent for Medical Treatment of Minors
(If you are under the age of 18, a parent/guardian must sign this form.) - Family PACT Client Eligibility Certification (CEC) Form
(If you are enrolling in Family PACT.)
*Although you will be completing a Health History form, you may want to contact your doctor/clinic for a copy of your medical record that you can bring to your visit.
