OTM Forms
OTMs can be filled out and submitted on the online database.

OTM form for downloading
1. Click on the form you need.
2. Click on "save this file to disk.
3. Save the file to the desktop, or any directory you wish.
4. Click on the file you downloaded to uncompress it.
5. Load the document into Word.
6. Fill it out and send it to RHA (see below).

OTM SUBMISSION
To submit completed OTMs you have multiple options.
- Drop off in the main housing office, in an envolop marked RHA.
- Drop of in the Community Directors Boxes in the Park offices.
- Email to to csun_nrhh@yahoo.com
- Drop of in the RHA Community Center located in Building 9
You will recieve a confirmation that your OTM was recieved via email.
