Individual Project: Final Project Report
The following elements are to be addressed in this fourth and final individual project paper:
1. Introduction and Overview of the Project: (15 points)
- Briefly review your individual project including the specific health-damaging behavior you have been working with.
- At a minimum, five current references should be included in your review of the health-damaging behavior you have been working with.
2. Needs Assessment: (25 points)
- Identify and discuss the methods and tools that were used to conduct the needs assessment (Formative Evaluation) component of your project.

Present the results of your needs assessment including the causal chain(s) targeted for your health behavior change interventions. Please be thorough in your presentation of your findings.
3. Interventions: (25 points)
- Present and thoroughly discuss all interventions that were employed to help your subject make the change(s) they were seeking in the project.
- Describe all tools which may have been included as part of your planned interventions with your subject. Be sure to include copies of these in the Attachments to this paper.
4. Evaluation Plan: (25 points)
- List and discuss the evaluation tools and any standards you used to assess the effectiveness of your interventions to promote the health behavior change(s) you were addressing in your project.
- Discuss data analysis techniques employed to determine success or failure of your intervention activities.
- Present the results of your evaluation measures using charts, tables, graphs, narrative or other reporting methods to describe your findings.
- Discuss the results of your findings including the implications of these results for future work you may do in the realm of health education and health behavior change.
5. Summary and Conclusions: (10 points)
- How does what actually happened compare to what you predicted would happen and what might you do differently the next time?
- Based on your experiences with this project, class readings, discussions, and exercises:
