Senior Project

Final Project Paper

The following outline should be used to develop the final project paper for Health Science 445.

1.   Project introduction and overview:

Please write a narrative overview of your group project. Be sure to include the following items:

• Description of how the project evolved.
• Where the project is to be carried out including the community and specific location the project will be implemented in.
• The specific target group to be the recipients of your program.
• The specific health issue to be addressed by your project; and
• The educational-behavioral diagnosis that was made to establish the need for your project.

2.   Project Goal(s), Health Education Program Objective, Health Education Target Group

      Objectives for Knowledge, Attitude and Behavior, and Learner Objectives for Knowledge,

      Attitudes and Behavior.

(5 points) 

3.   Methods and Activities:  

Please list and discuss all actual methods that were employed to carry out your project and the major activities that were needed to achieve your methods. You may choose to include a project timeline as a way of demonstrating all activities that were employed to carry out your project. Be sure to include the following as attachments to this paper:

• Curriculum used for teaching.
• Copies of educational materials handed out in your program.
• Other materials you feel would be helpful for the course instructor to see that support your Methods and Activities.

4.   Implementation Activities:

Please describe all implementation activities that were utilized to carry out your project. This may include any or all of the following:

• How you may have involved the target group in project planning activities?
• Marketing methods that may have been used (include copies of flyers, etc. in the Attachments to this paper).
• How the actual teaching of your Program was divided up among the various group members?
• What preparation activities were carried out prior to the actual day of implementation and your presentation?
• What, if any, changes might have had to be made the day you actually implemented your project?
• Anything else you feel is relevant to understanding what occurred in your implementation processes.
• Project Budget

5.   Evaluation:

Describe all evaluation activities that you utilized to assess the effectiveness of your Program to achieve its stated objectives. Please include the following:

• The results of all data gathered from your evaluation efforts. Data may be presented in narrative format and can be supported with charts, tables or other formats as desired.
• Your assessment of whether you achieved your stated objectives based on the data you presented above.
• All instruments used for carrying out your evaluation efforts (i.e., pre/post tests, satisfaction surveys, performance instruments, observation checklists, etc.) All instruments used must be included as attachments to this final paper.

6.   A group discussion of what was learned from carrying out this project including what,

      if anything, the group would have done differently had they to do this project

      all over again?

(5 points)

7.   Attachments labeled A, B, C. etc.

The final project paper is to be double-spaced and will be graded for content, grammar, spelling, punctuation and flow. Please include a Reference/Bibliography page if any citations are used in the paper.
