Group Project Paper #1

The following elements will need to be included and/or addressed in the first group project paper:

1.     Cover page with title of the project, name of all group members and semester.

2.     Table of contents with page numbers for all pages of the paper and a listing of Attachments.

3.     Introduction to the project.

4.     Phase I: Social Assessment:

• Identification of target area and geographic descriptors and map
• History of the target area
• Windshield Survey
• Demographics of the area
• Political structure
• Health Care System
• Community description (i.e., religion, schools, cultures, transportation, etc.)
• Social Assistance System
• Asset Mapping
• Key Informant Interviews
• Quick Intercept Interview data with Instrument in the Attachments to the paper
• Summary of Phase I data

 5.      Phase II: Epidemiological Assessment & Behavioral & Environmental Factors

• Epidemiological data presentation with tables, charts, etc.
• Problem listing based on epidemiological data
• Target group and issue to be addressed by the project
• Behavioral risk factors related to the problem & target group being addressed
• Environmental risk factors related to the problem & target group being addressed
• Changeability Matrix
• Summary of Phase II

 6.      Phase III: Educational & Ecological Assessment

• Predisposing, enabling & reinforcing factors
• Target group specific needs assessment data presentation (instrument to be included as an Attachment)
• Preliminary literature review of the problem
• Ecological & Educational Diagnosis

All papers are to be double-spaced and the use of subheadings is expected to help delineate the various sections of
the paper being written. Papers can be stapled, bound or placed in a three-ring binder.
