Dear Fieldwork On-Site Supervisor/Administrator:

Hopefully by now you have had an opportunity to meet with your staff member who is a candidate for both the Masterís Degree and the Preliminary Administrative Services Credential to discuss the Fieldwork (ELPS 688) course requirements.

One of the course requirements is that I, the University Supervisor, need to schedule a conference with you and the candidate that you are supervising on your site. To that end we have developed conference options and questions in order to meet the requirement and at the same time not impact even more your already busy schedule. For your convenience the questions are attached.

The conference options are:

Telephone Conference: to be pre-arranged for a specific time and date. Please note that for the telephone conference we will be asking that you use Pacific Standard Time. I would call the telephone number you provide me and call you at that pre-arranged time and date. The candidate will need to be present during the telephone conference.

E-mail: You may respond to the questionnaire via e-mail, discuss your responses with the candidate and then email your completed form to me.

Fax: Once you have completed the questionnaire for Fieldwork On-Site Supervisors you can fax it to my attention at 818-677-5720.

WebCam Conference: schedule a pre-arranged time and date on Pacific Standard Time. In order to accomplish this it is required that you have access to a webcam and microphone. Additionally, I would need to be provided a telephone number where I could reach you during the scheduled webcam conference in case of technical difficulties.

In all of the above instances the information provided to me will need to be shared with the candidate if he/or she is not present during the conference, etc.

Please feel free to choose one of the options that best suits your schedule. We ask that this task be completed by December 5, 2008.
