Recently, the American Diabetes Association has defined new diagnostic criteria*:
Symptoms + casual BG > 200 mg/dl (11.1 mmol/L)

OR

FBG > 126 mg/dl (7.0 mmol/L)

OR

2-h BG in OGTT value > 200 mg/dL *

*Testing must be repeated on a separate day; FBG is preferred test. (BG = blood glucose; FBG = fasting blood glucose; OGTT = oral glucose tolerance test)

Rationale for the new diagnostic criteria include:

  • FBG and 2-h BG levels represent approximate thresholds for risk of microvascular and macrovascular disease.
  • FBG and 2-h BG cut-points define equal risk
  • FBG is simpler, easier, and cheaper test than OGTT
