 Historical information collected for the nutrition assessment includes medical, social, and dietary information that could impact the patient's health.
Examples of such information include alcoholism, diseases, eating disorders, family health history (obesity, DM, CVD), surgeries, mental impairment, weight changes, dentition, exercise habits, and medications. It is often helpful to use a form to compile historical information, especially when gathering the information from a patient interview. Much of the medical and social history information can be gleaned from the patient's medical chart.
