Type 1 Diabetes

C.M. is a 10 year old female patient in your hospital, diagnosed yesterday with type 1 DM. You performed a diet history in the presence of her parents. She complained of fatigue and has lost 20 pounds in the last couple of months, although she reports that she has been eating more than usual due to excessive hunger. Her current weight is 110 lbs and her height is 62 inches. CM's IBW is 120 lbs.

Before the increase in appetite, her usual intake consisted of a bowl of cereal with 1/2 cup milk and a glass of juice for breakfast, a whole sandwich with meat and cheese, chips, 3 cookies, and a cup of 2% milk for lunch, a snack of chips, cookies, or fruit after school, dinner consisting of a main dish (meat, fish or chicken-based) vegetables and sometimes salad or rolls, and an evening snack of ice cream, pudding or "whatever".

CM likes candy and chews bubble gum. She has a good attitude about a healthy diet, however, and her parents are motivated to help her by providing appropriate foods.

Her labs are as follows:

    Lab Value Normal
Fasting glucose 600 upon admit; 230 now 65-115
    Albumin 3.2 3.5-5.5
    Hemoglobin 17 11.5 - 16.5
    Hematocrit 50 35-48
    Urine sp. gravity. 1.1 1.005 - 1.030
    urinary glucose positive negative
    urinary ketones positive negative

Medications: Insulin, 2 injections a day of regular and intermediate acting (assume appropriate dosage prescribed by physician)
