• Service Learning Pre-assessment Survey (mandatory)

• Sign up for activities:
  1. Sign into your Moodle page

  2. Scroll to the bottom of the page and in the search courses box type: Institute for Sustainability and click on the course

  3. You will be asked to enter an enrollment key which is: volunteer
  

• Student Check-List

• Food Garden and Compost Site Liability Waiver

Each Student MUST Print the waiver form, sign, date, and put your phone number. Leave the instructor signature blank. If you are under the age of 18, you must have a parent/guardian signature. Deliver to our office, Santa Susana Hall, room 116. If possible, please help us to continue our sustainable practices by printing out forms double-sided! The CSUN printers have this option available.

• Student Time Sheet

Students must use this time sheet to keep track of activities and hours completed. You must get a signature from the supervisor of the service learning site after you complete each activity in order to have your hours verified.

• Student Hand Book

This handbook provides students with information and procedures regarding service learning.
