Departmental Forms

1) Advisement Form

Each semester every math major has a hold on his/her registration packet. In order to remove this registration hold you are required to meet with your major advisor in order to plan your future coursework. Please download a copy of the Advisement Form and secure your advisor’s signature after meeting with him/her during the announced advisement period (which occurs during the last month of each semester). The signed
form should then be submitted to the math department office, SN 114, in order to remove the hold on your registration packet.


2) Course Substitution or Waiver for Bachelor’s Degree Form

Use this form in order to obtain an advisor’s approval for course substitution or waiver of major or minor requirements for Bachelor’s degree.


3) Change of Undergraduate Major (Plan)/Minor for Currently Enrolled Students Form

Use this form if you would like to:
• Change your Major
• Add a New Major / Option
• Add a New Minor / Option
• Remove a Major
• Remove a Minor
Please take a completed form to your major advisor.


4) Application for Bachelor’s Degree and Diploma

After securing your advisor’s signature on this form, please submit this application to Admissions & Records during the appropriate filing period.


5) Second Bachelor’s Degree in Mathematics

Please follow this link to obtain information about earning a second Bachelor’s degree in mathematics. After downloading a copy of the form for the math option of your interest meet with a math advisor.
