Faculty, Staff, and Department sponsored events to be held on the CSUN campus must complete the Event Approval Form. CSUN Student events must contact the Matador Involvement Center at (818) 677-5111.

INSTRUCTIONS:

2. Fill out form completely
3. Fax completed form to: (818) 677-3017
