Reporting an Information Security Incident
In order to better serve the campus community and remain in compliance with various state and federal laws and regulations, the Information Security Office has developed some new procedures for handling an information security incident. For more information on determining if a security incident has occured and what to do if one has, download the full "Reporting an Information Security Incident" document.
Once you have determined that an information security incident has taken place, download the "Information Security Incident Report" form. For instructions on filling out the form, download the instructions document.
Information Security Incidence Response Procedures
The following document outlines procedures and guidelines for responding to CSUN information security incidents. This procedure allows for a coordinated response from Information Security, the Computer Security Incident Response Team (CSIRT), and others involved in investigation plus follow-up of reported information security incidents.
