Security Incident Reporting
Determining a Security Incident
In order to better serve the campus community and remain in compliance with various state and federal laws and regulations, the Information Security Office has developed some new procedures for handling an information security incident. For more information on determining if a security incident has occured and what to do if one has, download the full "Determining a Security Incident" document.
Security Incident Report (Form)
Once you have determined that a security incident has taken place, download the "Security Incident Report" form. For instructions on filling out the form, download the instructions document.
