PAR-Q FORM
(for people ages 15-69 years)

Start Here First!

Before anyone begins a physical activity program, it is important to assess whether you should check with your doctor before becoming much more physically active.

The PAR-Q is for people 15-69 years of age. If you're over 69 years of age and not used to being active, check with your doctor. After you fill out the PAR-Q, you'll know if you're ready to follow the calendar towards achieving HALF DOME. If your answers tell you to consult with your physician, you can show him the calendar so he/she knows what you'd like to do and you can follow his/her advice.

Many health benefits are associated with regular exercise, and the completion of the Par-Q is a sensible first step to take if you are planning to increase the amount of physical activity in your life.

For most people physical activity should not pose any problem or hazard. PAR-Q has been designed to identify the small number of adults for whom physical activity might be inappropriate or those who should have medical advice concerning the type of activity most suitable for them.

Common sense is your best guide in answering these few questions. Please read them carefully and check the yes or no opposite the question if it applies to you.
