Health Care Advance Directive Information
Health Care Directives are instructions specifying what actions should be taken for your health care in the event you are no longer able to make decisions due to illness or injury and appoints a person to make decisions on your behalf. This can also be a discussion with your family and physician about the kinds of care you want.

Use the Bliman Vascular Age Calculator to learn your cardiovascular age and the prevalence of cardiovascular disease in the United States.
The BVA Calculator is a risk assessment tool that combines fasting blood glucose, triglycerides, high-density lipoproteins, pulse pressure, and tobacco's vascular toll to give a composite index of cardiometabolic age and disease.

If you have a medical emergency, call 911 for immediate assistance. When the SHC is closed, medical care is available at local urgent-care centers or hospitals.
