Federal Direct Parent Loan for Undergraduate Students (PLUS) 2012-2013
Submit this form to apply for the federal direct parent loan for undergraduate students (PLUS). The deadline to request a 2012-2013 academic year loan is May 10, 2013. Requests for summer 2013 loans cannot be submitted until April 22, 2013.

Federal Graduate PLUS Loan Information and Request Form (GRAD PLUS) 2012-2013

Master Promissory Note (MPN)
The first time you borrow student loan funds through the Federal Direct Loan Program at Northridge, you must electronically sign a legal document called a Master Promissory Note through the Department of Education.

Unaccompanied Homeless Youth Verification Form 2012-2013
Submit this form if you indicated on the FAFSA that you are or were an unaccompanied youth who was homeless or at risk of being homeless. In addition, you must submit a signed copy of a homeless youth status determination letter from one of the following sources: a high school or school district liaison; a director of an emergency shelter or transitional housing program funded by the U.S. Department of Housing and Urban Development; or a director of a runaway and homeless youth basic center or transitional living program.
