Unaccompanied Homeless Youth Verification Form 2012-2013
Submit this form if you indicated on the FAFSA that you are or were an unaccompanied youth who was homeless or at risk of being homeless. In addition, you must submit a signed copy of a homeless youth status determination letter from one of the following sources: a high school or school district liaison; a director of an emergency shelter or transitional housing program funded by the U.S. Department of Housing and Urban Development; or a director of a runaway and homeless youth basic center or transitional living program.
