Submit this form to apply for the federal direct parent loan for undergraduate students (PLUS).

Federal Direct Graduate PLUS Loan Request Form 2014-2015
Submit this form if you're a graduate student and wish to apply for the federal direct graduate PLUS loan.

Federal Direct Stafford Loan Request 2014-2015
Do not submit this form if you’re able to accept your Stafford loan online (see the loan acceptance tutorial). 

Private Education Loan Applicant Self-Certification
If you have applied for a private education loan, you may have to submit this form directly to your lender.

Authorization to Release Information
Submit this form to authorize the Financial Aid & Scholarship Department to release student or parent information to a third party. This form must be completed every year. 

Confirmation of Update to Citizenship 2014-2015
Submit this form to confirm that you have updated your U.S. citizenship status with the Social Security Administration. You must also present, in person, to the Financial Aid & Scholarship Department your original citizenship document as proof of your U.S. citizenship status. Do not send original citizenship documents by mail.  For additional information, refer to How to Submit Proof of U.S. Citizenship guide.

Conflicting Parent Marital-Tax Filing Status Work Sheet 2014-2015
Parents of dependent students must submit this form when it is requested by the Financial Aid & Scholarship Department. Log in to the myNorthridge Portal, go to the SOLAR Student Center, and review your To-Do List to see if the form is requested.

Conflicting Student Marital-Tax Filing Status Work Sheet 2014-2015
Independent students must submit this form when requested by the Financial Aid & Scholarship Department. Log in to the myNorthridge Portal, go to the SOLAR Student Center, and review your To-Do List to see if the form is requested.

Dependent Student Verification Work Sheet 2014-2015

Independent Student Verification Work Sheet 2014-2015
Independent students must submit this form when it is requested by the Financial Aid & Scholarship Department. Log in to the myNorthridge Portal, go to the SOLAR Student Center and review your To-Do List to see if the form is requested.

Submit this form if your housing status changes.

Status Change Notification Form 2014-2015
Submit this form if your enrollment status changes.

Verification of Dependents Other than a Child or Spouse Form 2014-2015
Refer to your To-Do List request letter if you don't know whether to submit this form. The Financial Aid & Scholarship Department will request it if it's required. Refer to the Verification Guide for additional information.

Selective Service Registration 2014-2015
Submit this form to notify the Financial Aid & Scholarship Department of your Selective Service registration status. Male students between the ages of 18 and 25 are required to register with Selective Service before financial aid eligibility can be determined. You must attach proof of your Selective Service registration or proof of your exemption status. Log in to the myNorthridge Portal, go to the SOLAR Student Center and review your To-Do List to see if the form is requested.  For additional information, refer to How to Submit Proof of Selective Service Registration guide.

Statement of Educational Purpose 2014-2015
Submit this form so that the Financial Aid & Scholarship Department can verify your identity and collect a statement of educational purpose as required by the U.S. Department of Education. You must appear in person at the Financial Aid & Scholarship Department, present a valid government-issued photo ID and sign a statement of educational purpose attesting that the federal financial assistance you receive will be used for educational purposes only. If you are unable to appear in person, you must sign the statement, have a notary public sign it to confirm that you appeared and presented the ID confirming your identity, and submit it with a copy of your ID.
