This form is to be completed by students to notify CSAC of any changes in the student's name, SSN, address, school, withdrawal from the grant/fellowship program, or for requesting a leave of absence.

  • If your financial situation adversely changed because of unusual circumstances in calendar year 2009, submit this form between September 14, 2009 - April 16, 2010.
  • If student will be attending ONLY the fall 2009 term, you must submit your Income Appeal by November 20, 2009. If the unusual circumstance is due to divorce/separation or death of a parent, the form is accepted immediately.

Use this form to activate your Stafford Loan IF YOU ARE UNABLE TO ACCEPT IT ONLINE. You must accept your loan by the following deadlines:

  • Fall Loan - 12/11/2009
  • Spring Loan - 5/7/2010
Loans for the 2009-2010 academic year must be accepted by 5/7/2010.

Student and parent(s) must complete and submit this form in order to receive the Federal Parent Loan for Undergraduate Students (PLUS) Loan.

  • Students with a FAFSA on file at CSUN may see their 2009-10 Cost of Attendance and Estimated Financial Assistance by visiting the “myNorthridge Portal” and clicking on Student Center, then View Financial Aid, and lastly click on Financial Aid Summary.
  • Students without a FAFSA on file may see a sample 2009-10 Cost of Attendance, for their degree objective, by clicking on http://www.csun.edu/finaid/cost10.html
  • Students may also have this form completed by a Financial Aid Representative. Please visit Financial Aid at the Student Services Center on the 1st floor of Bayramian Hall, west of the Oviatt Library.

Complete and submit this form if your enrollment status changes, or if your housing plans changed from when you filed your financial aid application (FAFSA).

Use this form to notify the Financial Aid & Scholarship Department that you have completed the on-line TEACH Grant Counseling and Agreement to Serve (ATS) requirements in order to have your TEACH Grant eligibility reviewed.

Adobe Acrobat PDF document.Unaccompanied Homeless Youth Verification Letter 2009-2010 Complete and submit this letter if you indicated on your FAFSA that you are/were an unaccompanied youth who was homeless or you were self supporting and at risk of being homeless. To verify your status, you must submit letter as completed by the appropriate homeless shelter.
