Meal Plan Cancellation Request

 

PLEASE READ THE CANCELLATION POLICY BEFORE SUBMITTING THIS FORM.
Please do not submit this form if your request does not fit any of the cancellation options listed below. You must speak to a Meal Plan office staff regarding your request first.

Submitted cancellation requests are subject to an approval review. In order for your request to be approved, we must be able to verify your reason for cancelling. Because other departments are involved in the cancellation process, please allow up to 4-6 weeks for approved cancellations to be processed. Please note that $50 of the $160 Initial Meal Plan Fee that was paid with the meal plan application is non-refundable.  

Once the Fall 2017 semester has begun, approved cancellations are subject to a $60 cancellation fee plus a charge for the number of weeks enrolled in the Meal Plan program up to the date of cancellation regardless of plan usage.
