Address Correction or Change Form - You may print and complete this form or update your home address, phone number and other data online in the Student Center (SOLAR).
SOLAR Navigation Path: Using Internet Explorer browser version 5.0 or higher, log into myNorthridge at www.csun.edu with your CSUN User ID and Password. Then select My Path to Graduation › Student Center (SOLAR) › Personal Information.
Application for Bachelor's Degree and Diploma Form - Undergraduate students expecting to graduate within a year, should submit an Application for Bachleor's Degree and Diploma form. ($47.00 processing fee).
Bachelor's Degree Date Change - Undergraduate students who have already applied for graduation may change their date by submitting this form. ($8 processing fee)

Change of Major or Minor Form - Currently enrolled students who are not ready to apply for graduation should use this form to change their major and/or minor.
Duplicate Diploma Request Form - Student's may request a duplicate copy of their diploma. ($8 processing fee)
Substitution or Waiver Form - Students who have applied for graduation may alter their departmental evaluation with approval from their major department.
