This exemption is available to certain nonresident students who meet a specific set of requirements as outlined in the instructions. Also include a completed CSU Residency Questionnaire along with your exemption request.
You may update your address and emergency contact information online through the SOLAR Student Center in myNorthridge. Show me how. Alternatively, print and file this form with Admissions and Records. 
Undergraduate and graduate applicants to CSUN can forward their application to a new term in CSUMentor without incurring an additional application fee. Select the link for instructions.
Students may request that a confidentiality hold be placed on, or removed from, their student records.
Undergraduate students enrolled at one of the California State University campuses may enroll at one of the University of California campuses or a California Community College on a space available basis. (Specific policies and procedures for eligibility are explained on page 2 of the form.)
Diploma Duplicate

Use this form to request a duplicate bachelor's degree diploma ($8 processing fee).

Previously disqualified undergraduate and second bachelor's students: To be considered for readmission to CSUN, submit this questionnaire along with transcripts of any courses completed and in-progress at another college or university for review by the major department and the Office of Undergraduate Studies. For Fall 2014 admission, file no later than April 1, 2014.
Undergraduate students: If you have specific questions regarding your Degree Progress Report (DPR) and you have NOT received a Graduation Evaluation, you may submit a DPR review request. A response will be sent to your CSUN email address.
Approval for extra units over the fall or spring maximum unit load for students in good standing is strictly limited at this time. Approved forms will be processed when Nonrestrictive Registration begins.

Undergraduate Students: Obtain approval of the associate dean for your major, or the Associate Vice President of Undergraduate Studies if undeclared.

Graduate or Credential Students: Obtain the approval of your graduate coordinator.

Approval to enroll in more than 13 units in summer term (all sessions combined). Form will be processed by Admissions and Records after Nonrestrictive Registration begins.

Undergraduate applicants may request to waive the application fee for fall or spring admission to CSUN and certify United States citizenship or immigration status.
International students: The affidavit is necessary for all F-1 (Certificate of Eligibility for Nonimmigrant, Form I-20) and J-1 (Certificate of Eligibility for Exchange Visitor, Form IAP-66) applicants.

Graduate Studies Forms Graduate students: Visit the Office of Graduate Studies website to view graduate student forms.
Undergraduate students: File this form to apply for the bachelor's degree and diploma when you expect to graduate within one year. Include the $47 processing fee (an additional $10 late fee will be assessed after the deadline). View application deadlines and instructions.
Undergraduate students who have already applied for graduation may change their date by submitting this form together with the $8.00 processing fee.
Students requesting a grade of Incomplete ("I") should use this form. An "I" may be assigned when a portion of the required course work has not been completed and evaluated in the prescribed time period due to unforeseen, but fully justified, reasons and that there is still a possibility of earning credit.
Students requesting an extension of time to complete work for a course in which they have received a grade of incomplete ("I") should submit this form PRIOR to the end of the calendar year which follows the original assignment of the "I" grade.
Students requesting approval to attend two CSUs concurrently in the same semester should complete this form, review the instructions carefully, and submit it to their home campus.
Students requesting approval to attend a CSU other than their home campus for one semester should complete this form, review the instructions carefully, and submit the application to their home campus.
Admissions and Records may accommodate a request for a letter when students have a special need that is not met by a standard university document.
Currently enrolled undergraduate students may change their major (plan) with departmental approval using this form or by filing an online request (see the guide Change Major/Minor Online). Important! Consult with an advisor to determine how the change could affect the completion of degree requirements. Before you request a change online, ask your advisor if pre-advising is mandatory to evaluate your request.

Complete this form and submit in person with valid photo ID. NOTE: If you have applied for graduation, your last name and first name on your diploma must be the same as your current name on record.
Use this form to correct Social Security Number, date of birth or other demographic information. Submit completed form to Admissions and Records.
Students seeking to petition the use of undergraduate coursework for postgraduate credential credit or to request Academic Renewal may use this form ($10 processing fee). Please refer to the instructions on page 2 of the form for university policy and procedures. For GE or other modifications, please submit a DPR Review Request.
If you feel that an error was made on a Change of Program Form or in your academic records, complete this form and submit to Admissions and Records.
Undergraduate students must use this form to request the approval of their Associate Dean to enroll in a class for the third time (i.e., repeat the class a second time).
CSUN applicants completing the CSU Residency Questionnaire will ensure proper determination of their residency status for tuition purposes pursuant to Education Code Section 68041. You may submit additional information that you believe will establish your California residency.
Continuing CSUN students may request a reclassification of their California residency status.

Official transcripts of courses taken at CSUN are issued only with the written request of the student. Facsimile transmissions are not accepted. You can order transcripts online (additional fees apply), by mail or in person. See How To Obtain a Transcript.
For students who have been accepted for consideration at one CSU campus, but now want to be considered by another CSU campus (same term only).
ONLY continuing students who will have completed 75 units at the end of Fall 2013, but have not yet attempted the UDWPE, will have a registration hold for Spring 2014 enrollment. To remove the hold, you must file an appeal using this form (online or paper version).

Late Add/Drop Forms - Change in Academic Schedule for Undergraduates

4th Week Change of Schedule Request for Undergraduate Students All undergraduate and second bachelor's students requesting schedule adjustments in Week 4 of the fall or spring semesters should use this form. This form will NOT be accepted BEFORE or AFTER the 4th week of classes. Also use this form for partial or complete medical withdrawals in Week 4. See Adjusting Your Schedule for deadlines.
All undergraduate and second bachelor's students requesting schedule adjustments during Weeks 5-12 of the fall or spring semesters should use this form. Extraordinary circumstances ONLY will be considered. For partial or complete medical withdrawal,submit this form together with the "Medical Withdrawal: Student Consent and Health Care Provdier Guides."
All undergraduate and second bachelor's students requesting schedule adjustments during Weeks 13+ of the fall and spring semesters and thereafter (i.e., retroactive changes) should use this form. Extraordinary circumstances ONLY will be considered. For partial or complete medical withdrawal,submit this form together with the "Medical Withdrawal: Student Consent and Health Care Provdier Guides."
Medical Withdrawal: Student Consent and Health Care Provider Guidelines All undergraduate and second bachelor's students requesting a partial or complete medical withdrawal after Week 4 must complete this form and submit it together with either the Late Change in Academic Schedule for Weeks 5-12, or the Last 20% of Instruction and Retroactive Change for Weeks 13 and beyond. The Medical Withdrawal form acts as the student's signed authorization to disclose health related information to necessary areas on campus and provides specific guidelines for the health care provider to verify the student's condition.
