Undergraduate Degree Services Forms

  • Address Correction or Change Form - You may print and complete this form, or update your home address, phone number and other data online. Here's how:

    Log into myNorthridge at www.csun.edu with your CSUN User ID and Password. From the Home tab in the Quick Links box, select SOLAR Student Center > Personal Information.
  • Application for Bachelor's Degree and Diploma Form - Undergraduate students expecting to graduate within a year, should submit an Application for Bachleor's Degree and Diploma form. ($47.00 processing fee).
  • Bachelor's Degree Date Change - Undergraduate students who have already applied for graduation may change their date by submitting this form ($8 processing fee).
  • Change or Declare a Major - Currently enrolled undergraduate students who are not ready to apply for graduation should use this form to change or add a major.
  • Change or Declare a Minor - Currently enrolled undergraduate students who are not ready to apply for graduation should use this form to change or add a minor.
  • Duplicate Diploma Request Form - Students may request a duplicate copy of their diploma ($8 processing fee).
