Application Fee Waiver (2014-2015)
|Admission Application Fee Waiver form for Fall 2014/Spring 2015 and Certification of United States Citizenship or Immigration Status form (Undergraduates Only).|
|Undergraduate students expecting to graduate within one year, should submit this form with the $47 processing fee (an additional $10 late fee will be assessed after the deadline). View application deadlines and instructions.|
|Applications for Undergraduates and Graduates can be forwarded to a future, open term and will not incur an additional application fee.|
|Students may request that a confidentiality hold be placed on their student records.|
|Undergraduate students enrolled in one of the California State University campuses may enroll in one of the University of California campuses or a California community college on a space available basis. (Specific policies and procedures for eligibility are explained on page 2 of this form).|
|Applicants completing this form will ensure proper determination of their residency status for tuition purposes pursuant to Education Code Section 68041. You may submit additional information that you believe will establish your California residence.|

Undergraduate Students: Obtain approval of the associate dean for your major, or the Associate Vice President of Undergraduate Studies if undeclared.
Graduate or Credential Students: Obtain the approval of your graduate coordinator.

Extra Unit Authorization - Summer (TBA)
|Approval to enroll in more than 13 units in summer term. Form will be processed by the Tseng College after Nonrestrictive Registration begins.
Undergraduate Students: Obtain approval of the associate dean for your major, or the associate vice president of Undergraduate Studies if undeclared.
Graduate or Credential Students: Obtain the approval of your graduate coordinator.
|Undergraduate students who have already applied for graduation may change their date by submitting this form. Submit form with the $8.00 processing fee.|
|A grade of incomplete ("I") may be assigned when a portion of the required course work has not been completed and evaluated in the prescribed time period due to unforeseen, but fully justified, reasons and that there is still a possibility of earning credit.|
|Necessary for all F-1 (Certificate of Eligibility for Nonimmigrant - Form I-20) and J-1 (Certificate of Eligibility for Exchange Visitor-Form IAP-66) applicants.|

|Master's, doctoral or credential students requesting a late change in academic schedule: Use this form beginning with the fourth week of the fall and spring semesters, for the fifth and subsequent weeks of the semester due to extraordinary circumstances after the deadline for late registration and schedule adjustments, and for retroactive changes after the semester ends. |Medical Withdrawal for Graduate Students||Master's, doctoral or credential students requesting any kind of medical withdrawal: Use the form and follow the procedures required by the Klotz Student Health Center.|
|4th Week Change of Schedule Request for Undergraduate Students||All undergraduate and second bachelor's students requesting schedule adjustments in Week 4 of the fall or spring semesters should use this form. |Medical Withdrawal: Student Consent and Health Care Provider Guidelines||All undergraduate and second bachelor's students requesting a partial or complete medical withdrawal in Weeks 5-12, or Weeks 13 and beyond, must complete this form. It acts as the student's signed authorization to disclose health related information to necessary areas on campus, and it provides specific guidelines for the health care provider to verify the student's condition.|
